Trevor Marshall bases his concern on the very small fraction of the population with granulamatous diseases such as sarcoidosis where local productionof 1,25-dihydroxyvitamin D (calcitrol) can leak into the serum and dysregulate calcium metabolism. He also discounts observational studies of the health benefits of UVB and vitamin D, and overlooks prospective supplementation studies that find a benefit for cancer and infectious diseases in the 1000-2000 IU/day range. William Grant will be submitting a letter to the editor of BioEssays in response to Mr. Marshall's article.
