Just sent them both to you. Make sure your atty or Insurance copmpany approves of them in case you have a claim. Also I get my insurance for teeth whitening from someone other then my store ins. Was wayyyy less. Let me know if you need help. Email me directly as I am having surgery row and only check my direct emails during recoups.

Soo, could I have a copy of the spray tan consent form? Also I notice you are in Fla also, our state health inspector was just in and did not like my tanning consent form, which one do you use?

I recently had a wacko give my employees a hard time on the 2nd to last day from package expiring, so I froze his account-haven't seen him to let him know, but I'm not putting up w/ his crap & neither should my girls.
