Does anyone have an example of an EFT Cancellation/Freeze Form?

Don't freeze. 30 days notice in writing to cancel.

I find that giving them the option to freeze will make the sale easier because even seasonal tanners will like the option of freezing when they don't use the package for $5.00/month. 98% of them will not freeze their membership when they don't use it eventhough 50% customers say they are seasonal and like the freeze option....they still won't freeze it.

Many have had bad experiences with Gym's that lock them or they don't use the membership and pay for it. Giving them no contract and the option to freeze makes the sale very easy. eventhough they won't freeze or cancel it.......so it's a win win situation.

but yes, cancellation with 30days notice.

Yeah but freezing gives them the chance to reap the benifits of half price tanning and VIP pricing then cutting you off from the revenue to offset that discount without any penalties.

No freezing.

I allow a freeze, but only 3 months per calender year. That way, they freeze it, and when freeze is up, it goes back to the regular fee. It keeps them from cancelling many times, and you end up getting alot of unused months after the freeze is off.

So to cancel or freeze you just need a sentence or two stating the cancellation or freeze? Can anyone provide a sample form? Thank you!

We allow to freeze as long as they like but a minimum of 1 month.

When they are on freeze this also freeze's any discounts as well
