Here is a generic one you can adjust to your procedures and salon.


I release and discharge (Your Salon Name) from all manners of actions, judgments, sums of money and demands whatsoever in law, in regards to Airbrush tanning.
I have been advised that the wear of airbrush tanning depends upon my skins age, texture and absorbency ability and the after care that is given. I release responsibility of (your salon name) should there be any immediate fading or loss of the airbrush design by signing this release forum.
I affirm that I have been advised of the airbrush procedures and that the products being used are FDA approved and give my permission to proceed with the airbrush application.
I also release (your salon name) and their employees from any responsibility in the event I may not like the procedure. I will discuss the situation if I am not completely satisfied the director of the facility and allow them to make arrangements for my satisfaction.
I am not allergic to any dyes, inks or cosmetics to my knowledge, except for the following______________________________.
I have been advised that I should not shower for at least 5 hours after the procedure.
I have been advised to wear loose fitting clothing after the procedure and have been told not to use harsh soaps or exfoliate when showering or bathing after this application.

Here is one we suggest our salon customers can use (we're a manufacturer of sunless products)...

Please read, understand and sign the following.

• Spray tanning is accomplished by application of a solution containing the active ingredient: DHA (Dihydroxyacetone). DHA is considered to be safe and has been FDA approved and in cosmetic use. The solution is sprayed on to the skin either manually or in an automatic spray booth. The manual method is a much more detailed, custom/hands-on method.
• You will enter a private room where you will remove your clothing. This is up to you and your level of comfort. You should wear a dark swimsuit or underwear. Spa briefs, bras and panties may be available upon request. The solution will wash out of clothing; it is best to wash the swimsuit or clothing worn as soon as possible after your session.
• You will then be sprayed. This process will take approximately ten (10) to fifteen (15) minutes. After spraying, your skin should be dry before putting your clothes back on and you should not bathe or sweat excessively for nine (9) hours. The solution will give you an immediate bronzing effect. The bronzing effect is a result of a coloring additive in the solution that will remain on the skin until you are actually tan. When you bathe, the coloring will come off to reveal your actual tan beneath.
• All people are different. All ingredients in the product used in this procedure are intended for cosmetic use and generally regarded as safe. There are, however, occasions where individuals may be allergic to one or more ingredients in the spray tan solution.
• Be advised there is a small percentage of people whose skin does not react favorably to spray tanning. For this reason, we do NOT advise being sprayed for the first time when your appearance is critical; e.g., wedding, special occasion, photographic session, modeling assignment, etc.
• Caution – Pregnant or nursing women should consult heir physician before using.
• Warning – The safety of this product has not been tested.
• Warning – This product does not contain a sunscreen and does not protect against sunburn. Repeated exposure of unprotected skin while tanning may increase the risk of skin aging, skin cancer and other harmful effects to the skin even if you do not burn.

This Process has been explained to me and I hereby agree to allow an Employee of ___________________________ to apply Spray Tan solution to my skin. To my knowledge, I have no medical condition or allergy which would preclude me from having this procedure done.
I agree to hold _______________________ harmless of any and all medical complications that may arise. I have been advised to discontinue use if any reaction occurs.

You have agreed to participate in a series of (Salon Name) sessions to achieve the ultimate in sunless tan on your skin. To help you best achieve your results, please read the following material.

1. (Salon Name) solutions, like any self-tanner will not prevent your skin from burning in tanning beds or sun.
Please take proper precautions when in UV light exposure.
2. For best results maintaining your color is possible with visits of every 5-10 days. Healthy, hydrated skin will hold tan longer and achieve color faster.
3. For best results, wait 12 hours after being airbrushed before showering. This allows the solution to fully develop and will ensure longest results.
4. The (Salon Name) Technician is trained to provide as even a tan as possible. However, it is possible you will find areas of your tan lighter than others. In most cases, these differences are minor and disappear after a couple of sessions
5. Because of the moisture agents in the mist, your eyes may feel itchy or scratchy but there is nothing in the mist that will cause them any harm.
6. All the components of the (Salon Name) tanning solutions have been used in cosmetics and food products for decades and have proven to be sate for the skin. If you had any adverse effects using self-tanning products or moisturizers you will probably experience similar effects with the (Salon Name).
7. We recommend that you hold your breath, while the technician sprays the areas of your face.
8. Many customers wear a disposable shower cap (provided for you by (Salon Name) to keep the solution from getting in their hair. Others tan without covering their hair. The solution can not penetrate hair follicles. There have been no cases where tanning solution has changed the color of anyone's hair.
9. If you have any history of asthma or respiratory conditions that could be aggravated by the use of the (Salon Name) system, due to DHA products. Please contact your physician before using it and be sure to advise us of any past allergic reactions
10. If you are pregnant you need a release form from your doctor stating you have their permission to be airbrushed. We do not airbrush women in their first trimester of pregnancy.
I the under signed, understand and will comply with all instructions and have been properly instructed prior to my (Salon Name) Session. I am using services at my own risk. I hereby authorize agents of the salon to perform such tanning procedures as may be deemed necessary advisable, and have provided them with the above information required. I hereby relieve (Salon Name), as well as agents hold them harmless from any liability involved in the use of the tanning process. The salon and their agents are not liable for injury to personal property or the loss or theft of any personal property.

IMO there are major flaws in all of the release forms above.

Ziti and KGlennCole the products being used in an airbrush delivery are not currently FDA approved only in topical form unless this just changed the past 2 weeks.

SunburstTans your eyes are your mucas membranes and some type of protection should be offered to your clients to protect them. DHA has never been approved with contact to your mucas membranes neither in spray form or lotion form.

We add that our customers are supplied with nose filters, iwear, and undergarments. We do not spray anyones ears as well.

These are just easy changes that should keep your clients informed.

I can't share our full release only because it belongs to Infinity Sun....but like Ziti said you can just adjust what was offered here.
